Difficulty sounding out words? Does the student like to read/to be read to? What makes reading difficult?

How does student do on spelling tests? Spelling in context? Does poor spelling block writing? Examples of spelling problems:
 
Comprehension
Can student describe what he/she reads? Rereads assignments? Summarize movies? Can student draw inferences, main ideas, and conclusions from text/speeches? Difficulty following oral directions?
 
Written and Oral Language
Vocabulary: Does student have difficulty expressing self and/or ideas either verbally or on paper? Makes substitutions, motions, drawings, or sounds in place of appropriate words?

Syntax: Does student have difficulty with sentence structure, grammar, punctuation? Are writing projects difficult to begin and disconnected? Does student stop and start when talking, often rephrasing self?
 
Math
What is student’s attitude toward math? Is there a gap between homework and test performance? Basic facts? Calculations? Understands fractions? Decimals? Percents? Is he/she able to complete word problems?
 
Study Skills
How are studying and note-taking abilities? How does student manage homework, working independently, managing own time?
 
Test Performance
How does student perform on tests? Knows information before test but is unable to recall it? Does better on multiple choice, oral, essays, or fill-in-the-blank?
